Step 1
Discuss your options with a member of our team
A member of our team is available to listen, answer your questions, and help you decide if Empower Physical Therapy is right for you. Call us anytime between 9-5 mountain time at 1-833-861-6161 or request an appointment below and we will call you back within a few business hours.
Step 2
Verify your insurance
Once we obtain your insurance policy information, we will conduct a preliminary check to determine how your insurance plan benefits might work and, if possible, schedule an appointment—all during the same phone call. If we need to investigate further, we will get back to you within 24 hours. Once your benefits have been confirmed, an Physical Therapy Scheduling Specialist will review the details of your insurance plan’s coverage and answer questions you may have.
Step 3
Schedule an appointment
If your insurance plan works with Empower and you’re interested in coming to one of our Outpatient Centers for an evaluation, we will schedule your initial appointment. We’ll help coordinate the details of your visit to make the process as easy and stress-free as possible. The Empower scheduler will make sure your appointment is an expert in treating your specific therapy need and will book the appointment at a location that is close to your home.
